Of all the contact forms I’ve ever used, this one is the best looking and easiest to implement. 5 stars!
Can i add or subtract fields easily with this form?

I would like to add two fields to the form…city/state and zip. I looked the as and added everything there but the something is missing. Is there a documented way of adding fields?

You must not only add it to the actionscript but you must also add it to the php file if you want it to print the new form values out into the email.
