collapsible contact form component

This is a component fully skinnable (with flex css styles) that can be used for your applications.

The component is completely abstract, so you can write your server-side logic (included in the purchased package is a php example).

Included in the package is also a simple contact form that you can use without the collapsing functionality.

Note: file size is for the preview example. The size of the components is, of course, much smaller.
