Clear fields button miss’s out one of the fields. It’s not clear which fields are required.
Thanks, Paul_Ferrie, I thought I fixed that… :-/
Looks to be another great form! Can’t wait to try it out!
Ok I tried it out and I am getting this error message from my activity window after I press send.
“scf_sendmail_v3.php?message=840 – not found”
Also the message=has a different number after I refresh and try again. I know my server can handle PHP as I use them all the time and the files are uploaded to the same directory. And it’s the Account Number field that doesn’t clear.
Well commented though.
That was it! Works perfect! Thank you for the help!
This is a great file! 5*’s! I am just diving into it and it’s great. Before I rack my brain too hard, I don’t need any drop downs, how do I have the form send to only one email address? Is there a way to control the tables distances? I don’t see any css to control it, can I just add it? I am going to try but I thought I’d ask the creator first. Great file, thanks again!
Great! You’re welcome! No CSS involved. It’s really basic. Just open in Flash and your can move things around as you wish right on the stage.
To make that modification you can do one of two things:
Easy way… In the help file it tells you how to add different email addresses. Add the same email address in each place where there would a different one in the actionscript.
Hard way…. Strip out the big chunk of actionscript that is for the comboBoxes. Then refer to the helpfile to see how to remove it’s instance name from the list of required fields. Remove the combo boxes from the stage. Then open the PHP file and remove lines 8 and 9 in the PHP file. And on line 14 add the email address you’d like all emails to be sent to.
Feel free to send me a message through my profile page if you hit a snag in that process…
It looks cool but how would I change the following:
+ remove the “Required” button + hide the email form + remove account number
+ redesign the blue balloon, on the right side.
thanks
Ps: if i could make those changes, would purchase it. Does it actually work 100%?
Bucki, detailed instructions are included to walk you step-by-step on how to make some modifications to this form. Just about everything you see above can be modified to what you want.
As long as you are using Flash 8 or higher you can simply click and delete the “required” labels. The same applies to the error message (balloon). It’s just a shape that can be easily moved or removed from the stage when you open the form in Flash.
Also, I’m not sure what you mean by “hide the email form”. The image above is just a preview so the actual form doesn’t really look like that… but if you mean you’d rather not have HTML mail, you just remove all HTML tags included in the PHP file. If you need assistance with figuring out where these tags are, I wouldn’t mind helping you. Just click my profile and send me a message.
Also, you can rename the “account” number field to whatever you wish or you can even remove it.
And OF COURSE IT WORKS as long as you can use PHP on your server.
Hey Lovedoodle
Thanks for the reply mate
Yes I will purchase it soon… cos I need something like that.
However, do i need MySQL or just PHP runing website?! And one more thing:
To how many messages is the user limited to?! I mean, if the users sends constant emails, maybe the PHP site will not send it or even think its spaming… if u get me?!
Or does that not cause any problems?!
Thanks
Bucki, you do not need MySql. Just make sure you have PHP on your server. Your website itself can be a plain HTML site or entirely made in Flash, as long as your FTP server is allowed to use PHP mail protocol.
Also, the user will only be able to send one message at a time (unless they sit there and refresh the page numerous times after clicking the send button). See the preview for what I’m talking about.

Hello! This form is perfect for me!
But I need your help… How can I make the “Add to mailing list?”?.
I have your other form, more simple… i just copy and paste on fla. but on PHP I don´t know how to make this work.
Please help!
hello!
I have problems with the accents and special symbols and letters like portuguese ñ or ó ò é when I reveive the mail I see é… i dont know how to change the php, can you explain to me?
thanks a lot!
I want to say that the two problems above I already found the solution.
Now I am with another problem: when the email of the form is sent for a webmail with access through cpanel (horde), the email arrives with tags of the HTML inside of the body of the email.
Please, please help…
Are you sure that mail client (horde) supports HTML email?
Test your form in another mail client (like yahoo, or hotmail, or gmail) and see if the tags show. If the html tags continue to show, there may be a sytax error somewhere in the php file. Send it to me (copy and paste in the body of your email) and I can take a look at it for you.
Also, is there any way of keeping the ‘send’ message status for longer. It flashes by very quickly, but I’d like the user to be able to read the text once they have submitted the form.
Thank you.
Hi Lovedoodle
Felicitation for the work ! I would be interested with the same form but with the added captcha validation system Is it in your projects ?
Doumart
Hi,
it looks like a good script, but before purchasing it, i need to get some answers for my usage:
- Can it support Chinese (Traditional and Simplified) characters? - Can it be easily integrated in any existing website in term of layout?

Great form and code but whenever I alter ANY value in the drop-down fields, the form will only accept the letters ‘QERUID’ and nothing (including numbers/punctuation) else.
Puzzled???
I am having an email problem. On the form where a user would put there email it doesnt allow any email with a msn/yahoo/gmail or anyother domain, the only email domain that it allows is the domain where the form resides. HELP !!!!!!
I’ve sent you an email but I’ll add my suggestion here as well…
- Open up the php file and locate the Mail Headers section. - Locate the variable $sender in the From: section. - Replace that with an email address with the same domain name as the place where you are hosting this file. It does not have to be a real email address for this to work.
For now, try that and if you are still having issues, click my profile and send me a message. I’ll be able to assist you further.
Hi, 5 stars for this super work. Is possible to add a radio button in this template?
thanks
