Open .fla in flash 8+
set Flash Size in Properties tab
publish flash
set flash width and height in html

Open .fla in flash 8+
Click 2nd frame of main timeline
Select element on the stage
resize the input fields or drag the element to its position
publish flash

3.How to insert this form to other flash project?
In action panel, use loadMovie(“contactForm.swf”, yourSquare);
