Flash 8 Contact Form that sends email in HTML format

This contact form uses AS2 and PHP . Users can fill in required, and non-required text fields, select options from the comboBox, two sets of radio buttons, and a checkbox. All of these are sent to a single php file and then straight to the email of your choice. You can even send carbon copies and blind carbon copies to another email each time the form is submitted. But it’s not just a plain text email, this email is sent in HTML format. It’s quicker to read and easier on the eyes. Your web server MUST support PHP for this form to work.
